See the parallels to major accidents – Today’s Business

Oil

the Man who saved Statoil from the company’s worst gassutblåsning in 2004, believes “Songa Endurance”-event is similar.

the Article is added to your reading list.

At the end of november 2004, fought a small group of people for 15 hours against a gassutblåsning on the Snorre field in the north Sea. Boreplanleggingen on the land had failed completely in the forefront, and it was pure luck that the crew had stabilized the well when they only had a few cubic meters of drilling mud left in the tanks.

the Man who led the work, was then oim Dag Lygre. Now wakes the memories to live after Statoil for just over a week ago, put forward a investigative report of brønnkontrollhendelsen on the rig “Songa Endurance” on the Troll field in October. Lygre believes that this incident also reminiscent of the run-up to Bp’s catastrophic blowout on the “Deepwater Horizon” in the gulf of mexico in 2010, a deadly blowout that got totally out of control.

– I see distinct parallels. There was a ukultur on the installation which allowed that such things happened. A did shortcuts from selskapskrav. It was the same as happened on the “Deepwater Horizon” also, a had done such shortcuts before. It had been usual, ” says Lygre.

Also Statoil’s investigation refers to the Snorre A and the Deepwater Horizon in a summary comparison, although there are several differences. Statoil received nok 80 million in fines after the Snorre blowout. “Deepwater Horizon” cost BP around 62 billion dollars in cleanup, fines and damages.

– We see that there is a need for improvements on the Goblins both within the quality in risk assessments and change management. It goes on how we handle changes from the original plans, ” says Statoil spokesman Ola Anders Skauby.

” We believe that nevertheless there is no basis for asserting that there has been local ukultur or shortcuts. We do not see any general practice that deviates from Statoil’s stringent requirements for well control.

– There is a significant difference in these events. We had the barrier in place, that came after one minute.

Is it a possible cause of the Songa-event?

– We feel that it is streamlining and simplification that allows the wells are cheaper, not the shaving of the cost. The simplification and streamlining strengthen security, not weaken it. We have become better at planning and better at simplifying the well-design, ” says Skauby.

Investigation for “Songa Endurance”-blowout refers to several similar eruption.

  • “West Vanguard” in 1985, which had high gassrater on the deck.
  • “Treasure Saga” in 1989, with high gassrater on the deck.
  • Snorre A in 2004, which failed in boreplanlegging and exchange of experience, and resulted in 80 million in fines.
  • the Deepwater Horizon in 2010, where the de-sludging valve also was activated, then without working.
  • the Gullfaks C in 2010, where the drilling programme was changed without the documentation, and gave Statoil nok 30 million in fines.

“The Troll Way”

Lygre, who today works for the oil company Mærsk in Qatar, bases its assessment on a quick review of the report.

– the Field-teams at the Troll has over time developed a different operasjonspraksis from that which is used in the rest of the company, is there to read.

the Report describes how there had been an established routine on the Troll field, where a lot be to perform pressure measurements under certain draw operations even though this deviated from Statoil’s brønnintegritetsmanual.

Some of the interviewees have mentioned “The Troll Way” in a negative view, in the sense that “we have done such operations many times, they are standardized and simplified”. In this episode, it could have had a negative impact related to the awareness about the robust, independent barriers and hendelsesstyring, said the report.

the Report describes also how the crew of “Songa Endurance” had no experience with using the type vertical valve trees that were used. None the less chose to use a new type of valves as a barrier, a type that proved to work poorly in this setup. The communication between the people involved get rapids in the report. Important participants were not invited in risikovurderingsmøter.

– This is one of the most serious brønnkontrollhendelsene we have had in Statoil,

Gassutblåsningen on the “Songa Endurance” were, however, stopped after a minute thanks to that barrier number two, boresikringsventilen on the seabed, worked. Admittedly struggled Statoil for the next ten days to drive gas out from the places it shouldn’t be in the brønnbanen.

Lygre think it is reassuring to see that the group is so open about the incident. He believes it would have been unthinkable for companies to publish such reports, for example, in the arabian Gulf, where he now works. He also emphasises that the Songa-the incident never escalated, thanks to barrier number two. The highest agent concentration, which was measured on the rig, was at 60 percent of the lower eksplosjonsgrensen.

– There is a very low level. They managed to stop this before it was directly life-threatening. It is, therefore, always work with two barriers. The other will turn into if the first fails. As such, it was within the design, and not very dramatic that happened, ” says Lygre.

Statoil spokesman Ola Anders Skauby said it is important for Statoil to transparency about such incidents, both for learning within and across the company. He refers to the company over several years has had a strong improvement in an indicator called serious hendelsesfrekvens, even if the arrows pointed the wrong way in the last year. He also rejects the Songa incident is directly related to Statoil’s efforts to streamline brønnoperasjonene.
